Program Fee Agreement

I understand that payments are due according to the Program Fee schedule. Segue Center reserves the right to dismiss a participant from the program in the event of physical aggression or disruptive verbal behavior. If dismissed, the participant would be responsible for the fees through the remainder of the quarter. For the Fall Program, the second quarter begins October 15, 2018. For the Spring Program, the fourth quarter begins March 15, 2019.

Release and Indemnity Agreement In consideration of the above-named participant being enrolled and permitted to attend activities at, make trips with, and participate in athletics with the Segue Center, and to the full extent allowed by law, I HEREBY AGREE TO WAIVE AND RELEASE SEGUE CENTER, its Trustees, Director, Administrators, Staff, agents, employees, volunteers, participants and invitees, (collectively referred to as “Releasees”) (but not third-party vendors and service providers, such as paid certified carriers), FROM ANY AND ALL CLAIMS, SUITS, LOSSES, DAMAGES, CAUSES OF ACTION OR OTHER LIABILITIES by reason of any accident or injury suffered by the above-named participant, which may arise in connection with center activities, and any of the authorizations described above, WHETHER OR NOT CAUSED BY OR ALLEGED TO BE CAUSED BY THE NEGLIGENCE OF THE SEGUE CENTER OR ANY RELEASEE. Further, I HEREBY ALSO AGREE TO INDEMNIFY AND HOLD HARMLESS RELEASEES from any and all claims, suits, losses, damages, causes of action or other liabilities, including but not limited to all damages and all expenses of litigation and/or settlement/release, by reason of any accident or injury suffered by the above named participant while at Segue Center, on program trips or participating in program activities (including meals and snacks), or athletics in connection with any of the authorizations described above, INCLUDING ANY ACCIDENT OR INJURY CAUSED BY THE NEGLIGENCE OF SEGUE CENTER OR ITS RELEASEES. Some participants may have very serious food allergies, such as to peanuts, tree nuts, dairy, gluten, and other foods. Segue Center attempts to protect participants with food allergies from foods that cause an allergic reaction. The terms of this paragraph expressly extends to and covers this risk. I HEREBY CONSENT for Segue Center to use photographs or other physical likenesses of the above-named participant for purposes of marketing/advertising/publicity/promotions in (1) print, (2) on the internet (Segue Center Facebook page), (3) Segue Center website, and (4) on public displays related to promoting the center. In accordance with Segue Center policy, names of individual participants will not be released with photographs without express written consent. I understand that I may revoke this photo release by submitting a written letter to the office.
