Registration Process

Step 1

Complete and submit the online Segue Center Application.

Step 2

Schedule an Initial Intake appointment with the Director. A visit may not be required for all applicants, but may be needed to determine if the applicant is a good candidate for our programs.

Step 3

Upon acceptance, the applicant will complete the online Segue Center Registration form. An invoice will be sent at the time of services.
